The Social Worker maintains a caseload of participants, conducting psychosocial assessments and developing care plans within an interdisciplinary team. They act as a liaison between participants, families, and care teams to ensure smooth care transitions and provide ongoing counseling and case management.
Candidates must hold a Master's degree in Social Work (MSW) and have at least one year of experience working with frail or elderly populations. Proficiency in care coordination and behavioral health support is preferred, along with the ability to obtain CPR and First Aid certification.
