Key Responsibilities

• will perform beside initial and ongoing case management assessments, counseling, referral and discharge planning services to all age groups of patients treated at Holy Cross; provides supportive services to patients and their families.

• coordinate the patients' discharge needs.

Requirements

• Requirements: State of Maryland LSWA, LGSW, LCSW or LCSW-C licensure (or license eligible) or RN licensed by the State of Maryland or Compact State license.

• Masters Degree in Social Work or Bachelor's in Nursing required.

• Previous experience in an inpatient setting.

• Demonstrated ability to work with all age groups of patients, with an emphasis on adult and geriatric patients.

Benefits & Compensation

• Salary: $70,158 - $105,227/year

Work Schedule

• Full-Time position.

• Day shift only - no nights.

• Flexible scheduling available.
