Hospice RN Case Manager

About the Role
Optimal Care Hospice Registered Nurses provide comprehensive nursing care, case management, and compassionate support to terminally ill patients and their families in home or facility settings. You will assess, plan, coordinate, implement, and evaluate individualized patient care plans while leading an interdisciplinary team.
Key Responsibilities
- Conduct initial patient evaluations and ongoing reassessments.
- Manage caseload including planning and evaluation of care plans.
- Perform hospice-specific assessments such as home safety and support system evaluations.
- Provide compassionate support to patients and families, including at time of death.
- Maintain professional communication with physicians, patients, families, and team members.
Requirements
- Graduate of accredited nursing program (Diploma, Associate, or Baccalaureate).
- Current Michigan RN license.
- Minimum 1 year clinical nursing experience.
- Ability to handle multiple priorities and documentation.
- Reliable transportation and valid automobile insurance.
