IHR Physical Therapist - Rehab Mobile Services

Key Responsibilities

The physical therapist will deliver outpatient physical therapy directly in patients' homes and utilize telehealth services to support patient care. They are responsible for managing the patient plan of care from initial contact to discharge while collaborating with the healthcare team.

Requirements

Candidates must hold a Bachelor's Degree in Physical Therapy and a valid Illinois state license. A BLS certification from the American Heart Association is also required upon hire.
