Key Responsibilities

The physical therapist will provide evaluative, consultative, and treatment interventions to home health patients in their place of residence. They are responsible for maintaining effective communication with the interdisciplinary team and documenting patient progress accurately.

Requirements

Candidates must hold a valid professional physical therapy license in the state of practice and be a graduate of an accredited physical therapy program. Prior home health experience is preferred.
