UnityPoint Clinic is a physician-led, nonprofit health care organization offering comprehensive services in primary care, walk-in care and specialty care. The clinical teams at the area’s clinics are improving outcomes, lowering the number of hospital readmissions and reducing avoidable costs associated with treating chronic conditions. Established team of 2 physicians and 4 Advanced Practitioners
Diabetes Self-Management program staffed with Certified Diabetes Educators (RN’s and Dietitians)
Healthy Living Program, a medically managed weight loss program supported by Dietitians and a Health Coach
Experienced staff, both clinical and non-clinical, who know the patients and the community
Full-time position, including dedicated administrative time
24/7 hospitalist program is responsible for adult inpatients
Fully integrated system with clinics and hospitals on EPIC EMR to improve care coordination
Responsibilities
Outpatient clinic only; Adult and adolescent patients 15+
Clinic hours Monday – Friday 7AM-5PM
No nights or weekends
Shared call amongst the providers. Triaged through MyNurse
Opportunity to work with patients with diabetes on an outpatient basis
Provide diabetes education and management to our patients and families
Focus on planning, supervising treatments, and monitoring diabetes related health concerns
Evaluate and treat multiple endocrine diseases
Work in collaboration with Diabetes Educators from an ADA Accredited Diabetes Self-Management program
Coordinating care of patients and working in collaboration with the patient’s primary care provider
Exercises independent judgements in the assessment, coordination, management and evaluation of certain medical processes and procedures, under the guidelines of the scope of practice and medical protocols
Ability to work as a team member to improve the health of the people and communities we serve
Qualifications
Nurse Practitioner or Physician Assistant
Board Certified
Endocrine experience preferred
Hold an unrestricted medical license in the state of IA
Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioners field of practice
Eligible to work in the United States
