Home Base Social Worker (LCSW/LICSW)

Key Responsibilities

This role involves collaborating with professionals to evaluate patient conditions and assess client needs as part of an interdisciplinary team, developing and implementing patient-centered treatment plans. Essential functions include providing psychosocial assessments, formulating disposition and treatment plans, and monitoring client progress against measurable goals.

Requirements

Candidates must possess a Master's Degree in Social Work and current professional licensure in Massachusetts (LCSW required, LICSW preferred). Preferred qualifications include training in trauma-focused modalities like Prolonged Exposure or Cognitive Processing Therapy, and experience working with Military service members or Veterans.
