Social Worker Case Manager I

Core responsibilities: Conduct biopsychosocial assessments and coordinate discharge planning to ensure a successful transition from acute to post-acute settings. Act as a patient advocate and resource person for counseling and end-of-life decisions while collaborating with interdisciplinary care teams.
