Key Responsibilities

• Work in partnership with young adults, adults, and their families/carers to assess needs and develop outcome-focused support plans.

• Undertake statutory assessments, safeguarding activity, and care planning, ensuring wellbeing and safety is prioritised.

• Assess risk and deliver interventions to improve quality of life and long-term independence.

• Coordinate multi-agency services, working closely with health, education, housing, and partner organisations.

Requirements

• To be successful in this role you must have;.

• Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.

• Registration: Must be registered with Social Work England.

• Experience: Experience within a statutory setting working with young adults and/or adults, including safeguarding.
