About the Role

Join Liquid Personnel as a Occupational Therapist – Reablement and Hospital Discharge in United States. Our client is seeking a skilled and dynamic Occupational Therapist to join their Reablement and Hospital Discharge pathway in Wiltshire. The role has arisen due to an increased workload linked to the successful implementation of the Home First contract.

Key Responsibilities

provide community-based interventions aimed at enabling individuals to return home safely and independently.

Conduct home and community visits, assessing functional needs Develop goal‑oriented, person‑centred reableme.

Provide evidence-based interventions to promote independence Maintain accurate, timely documentation Contrib.

provide appropriate support to you throughout the process and into your work placement.
