This position offers day shifts only. Work with Summa Health at Home and Hospice in Akron, Ohio.
Provide comprehensive skilled physical therapy services to patients within their homes as ordered by physicians. Develop and implement individualized plans of care while documenting progress and coordinating with the healthcare team.
Requires a current Physical Therapist license and at least one year of experience in an acute setting. Must possess a valid driver's license, auto insurance, and the ability to meet physical demands of home-based care.
