Rehab Director- Physical Therapist Assistant

Key Responsibilities

Evaluate patients and implement personalized physical therapy treatment plans to improve mobility, strength, and functional independence for older adults. Collaborate with interdisciplinary teams and caregivers to ensure continuity of care and track patient progress through clinical documentation.

Requirements

Candidates must be a licensed Physical Therapist or license eligible and a graduate of an accredited Physical Therapist Program. Interest or experience in skilled nursing, long-term care, or geriatric rehabilitation is preferred.
