Key Responsibilities

The physician will provide direct primary medical care to program participants, including performing physical examinations and managing episodic illnesses. They are also responsible for coordinating hospital admissions, nursing home care, and discharge planning while participating in team meetings.

Requirements

Candidates must hold a Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) degree and possess a current South Carolina medical license. A minimum of 2-3 years of experience, including at least one year working with a frail or elderly population, is required.
