About the Role

Join AccentCare as a Hospice Social Worker in Loveland, Colorado. This PRN role offers $35 to $45 per hour, providing compassionate social work services in hospice care. You'll support patients and families emotionally and spiritually, helping them navigate end-of-life challenges.

Key Responsibilities

• Assist hospice team in assessing social, spiritual, and emotional patient needs.
• Participate in interdisciplinary care planning.
• Provide grief counseling and discharge planning.
• Support bereavement programs.

Requirements

• Master's Degree in Social Work or related field.
• Licensed LSW or LCSW in Colorado.
• Valid driver's license for travel.
• Prefer 2+ years healthcare social work experience.
