Join Penn Medicine Hospice as an Admission Registered Nurse providing compassionate care to patients and families across multiple Penn Medicine facilities in Pennsylvania. This full-time role involves coordinating hospice admissions, performing assessments, and collaborating with interdisciplinary teams to ensure quality care.

Key Responsibilities

• Facilitate patient and family entry into hospice services.
• Perform hands-on assessments and determine appropriate care levels.
• Collaborate with healthcare teams to coordinate care across the continuum.
• Educate primary caregivers on immediate care needs.
• Practice within hospice regulations and reimbursement criteria.

Requirements

• Registered Nurse license in Pennsylvania.
• Bachelor of Arts or Science degree.
• More than 15 months of relevant nursing experience.
• BLS/CPR certification.
• Valid driver's license and car insurance.
