About the Role

Join El Rio Community Health Center as a Pharmacy - Pharmacist (Elk Ridge)- Grand Canyon-Pharmacy in United States.

Key Responsibilities

Provide comprehensive medication management, including medication histories, reconciliation, adherence assessments, and patient education.

Deliver evidence based, patient centered drug therapy recommendations and collaborate with the interdisciplinary care team.

Evaluate and verify medication orders for accuracy, appropriateness, dosing, interactions, contraindications, and duplicate therapy.

Identify, prevent, and resolve medication related problems; support patient safety, quality improvement, and clinical guideline initiatives.

Requirements

PharmD or equivalent doctoral degree – Required

3–5 years outpatient or retail pharmacy experience – Preferred

Active Arizona Pharmacist License – Required upon hire
