About the Role
Join El Rio Community Health Center as a Pharmacy - Pharmacist (Elk Ridge)- Grand Canyon-Pharmacy in United States.
Key Responsibilities
- Provide comprehensive medication management, including medication histories, reconciliation, adherence assessments, and patient education.
- Deliver evidence based, patient centered drug therapy recommendations and collaborate with the interdisciplinary care team.
- Evaluate and verify medication orders for accuracy, appropriateness, dosing, interactions, contraindications, and duplicate therapy.
- Identify, prevent, and resolve medication related problems; support patient safety, quality improvement, and clinical guideline initiatives.
Requirements
- PharmD or equivalent doctoral degree – Required
- 3–5 years outpatient or retail pharmacy experience – Preferred
- Active Arizona Pharmacist License – Required upon hire
