About the Role

Join Trinity Health as a Counselor Social Worker in United States. Employment Type: Full time Shift: Day Shift Description: COUNSELOR SOCIAL WORKER SUMMARY: Provide evidence-based individual, group, family, and educational services including assessment and psychotherapy. Document clinical and consultative activities in patient electronic medical record.

Key Responsibilities

WILL DO: Conduct evaluation of patients and collaborate with healthcare team members to support the emotional, mental, environmental, bio-psychosocial, and spiritual needs of the patient and/or their family/caregivers, as applicable.

Provide evidence-based individual, group, family, and educational services including assessment and psychoth.

Requirements

Licensed Clinical Professional Counselor (LCPC) or Licensed Clinical Social Worker (LCSW) in the State of Idaho One to three years of experience in a mental health setting required.

Experience in multiple therapy techniques preferred.

Ability to clearly document at each visit objectives, measurable goals and progress toward goals and identify the expectations of counseling with patients.
