Key Responsibilities

• Evaluates patients for initiation, change or discharge of plan of care.

• Develops and implements treatment plan incorporating treatment methods and goals with patient/caregiver input.

• Completes relevant documentation and record keeping of stated services and consultation with other disciplines and services per regulatory guidelines.

• Designs, fabricates, fits and educates in use of assistive devices, special equipment, splints and orthotics.

Requirements

• X Lifting/Carrying (Patient) Less than 25 lbs.

• X Pushing/Pulling 0 – 25 lbs.

• X Populations Served While performing this job, the employee may care for patients in the following age groups.

Benefits & Compensation

• Hourly rate: $57.75/hr.

Work Schedule

• Full-time position.

• Call responsibilities required.

• Weekend coverage may be required.
