The Nurse Practitioner is responsible for providing preventive medical, palliative, and educational services to promote patient health and wellness, conducting comprehensive medical assessments, treating acute and chronic conditions, and ordering/interpreting diagnostic tests. Essential activities include 100% focus on Care Management, which involves patient examination, treatment planning, counseling, referrals, communication with staff/families, and accurate documentation.
Candidates must possess a current unencumbered Certified Nurse Practitioner (CNP) license for Ohio, along with a Certificate of Prescriptive Authority or Externship Certificate to Prescribe. Required experience includes two years in the home health and/or hospice field, proficiency in Windows and Microsoft Office, and the ability to meet physical demands including driving and lifting up to 50 pounds.
