Help
The Social Work and Nursing Case Management Service Line is a multidisciplinary team that addresses the healthcare needs of veterans receiving Inpatient Care, Outpatient Primary and Specialty Care, as well as Extended Care Services. As a member of the interdisciplinary team, you will participate in the development and application of directed clinical treatment interventions. Functions of this position include, but not limited to:
Evaluates care and outcomes to ensure timely and appropriate provision of services.
Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care, patient and family education, patient self-management, and supporting factors that impact customer satisfaction.
