Key Responsibilities

Participates in intake process and develops initial and annual social service summary on each person receiving services.

Develops a treatment plan and goals based on information in summary.

Provides social services as prescribed by treatment plan in areas of counseling and concrete services including housing and residential placement, food stamps, Medicaid, half

fare cards, camp, vacation referrals, etc.

Requirements

Flexible 30 hours per week.
