Key Responsibilities

• provide integrated care to all ESRD patients, who are some of the most medically complex and vulnerable pati.

• manage care for more than 20,000 patients with late-stage chronic kidney disease (CKD) and end stage renal.

• provide great care management but we start with our heart with our patients and each other.

• manage comorbid conditions while addressing medical, social, emotional, and financial needs.

Requirements

• additional state licenses could be required Valid driver’s license and insurance in the state Intermediate computer skills (MS Word, Excel, Outlook) Strong clinical judgment, autonomous decision

Benefits & Compensation

• Salary: $118K - $150K
