Key Responsibilities

• · Demonstrate adherence to the PHP/SBHI Patient Experience philosophy, guidelines and tools.

• · Demonstrate strong ethics, honesty, and adherence to PHP/SBHI Code of Conduct and PHP/SBHI Compliance Program.

• · Schedule, provide and document all clinical services accurately and according to departmental standards utilizing and documenting evidenced based practices.

• · Collaborate with referral source, internal team members and external educational and service providers to ensure appropriate patient care and continuity of care.

• Full-Time position.

• No weekends required.

• Day shift only - no nights.
