About the Role

Join Andwell Health Partners as a Medical Social Worker – Hospice Care in United States.

Key Responsibilities

Assesses and identify the client’s holistic health care needs/concerns as part of a family and community.

Assesses social/emotional factors that are related to the patient’s illness, need for care, response to treatment, and adjustment to care.

Develops a plan of care under physician orders with realistic/measurable outcomes in collaboration with the client/family and health care team providers according to Organization protocols.

Implement patient care plan activities based on treatment goals towards independent living for the patient/family.

Work Schedule

Full-Time position.

Candidates must possess a degree from a social work school accredited by the Council on Social Work Education and hold at least an LSX licensure from the Maine Board of Social Work.
