Registered Nurse - Remote Patient Monitoring - Home Telehealth Care Coordinator

About the Role

The Department of Veterans Affairs Greater Los Angeles Healthcare System seeks Registered Nurses for Remote Patient Monitoring and Home Telehealth Care Coordination. Manage complex patients using telehealth technologies to improve outcomes.

Key Responsibilities

• Coordinate care for high-risk patients via Remote Patient Monitoring.
• Perform comprehensive clinical assessments and develop individualized care plans.
• Monitor biometric data and facilitate timely interventions.
• Collaborate with interdisciplinary teams including mental health, pharmacy, and rehab.
• Provide health coaching and disease-specific education.
• Lead quality improvement initiatives.

Requirements

• Graduate of an accredited nursing program.
• Current, full, active RN license in any US state or territory.
• English proficiency.
• Ability to pass background and physical exams.
