Key Responsibilities
- Review discharge summaries and conduct remote consultations with patients, caregivers, providers, and/or outside pharmacies to complete post
- discharge medication reconciliation
- Review and analyze medication orders and patient profiles to identify and resolve medication
- related issues.
Requirements
