About the Role
This part-time Clinical Pharmacist role with Medical Home Network is fully remote, working approximately 16 hours per week. You will serve as a patient-facing provider and clinical resource focused on medication management, patient education, and chronic disease support.
Key Responsibilities
- Manage medications and optimize patient outcomes.
- Educate patients and families on medication use.
- Support chronic disease management.
- Embed pharmacy expertise into partner practices.
- Improve medication safety and adherence.
- Reduce hospital admissions and readmissions.
- Enhance quality performance in value-based care models.
