About the Role

BAYADA Home Health Care seeks a full-time Occupational Therapist for home health visits in Louisburg, North Carolina, serving Granville, Vance, and Franklin counties. This role involves providing rehabilitative care to adult and geriatric patients in their homes.

Key Responsibilities

• Conduct home visits to evaluate functional status including muscle function, endurance, and ADL/IADL.
• Assess cognitive, psychological, and motor performance to develop intervention plans.
• Evaluate home environments for hazards and recommend equipment.
• Collaborate with nursing and multidisciplinary teams to revise care plans.
• Educate patients and families on rehabilitative care and safety.
• Document observations and interventions using electronic medical records.

Requirements

• Current North Carolina Occupational Therapist license.
• Minimum one year recent OT experience.
• Graduation from an accredited occupational therapy program.
• Ability to work independently with strong organizational and interpersonal skills.
• Computer proficiency; EMR experience preferred.
