Key Responsibilities
- provide whole person care addressing bio-psycho-social needs of patients and family.
- Provide patients/families with the psychosocial support needed to cope with chronic, acute or terminal illne.
- provide education about homecare, palliative care, and hospice.
- provide proof of license within 4 months of start.
Requirements
- If LGSW, must be actively working on obtaining MN licensure at the LISW or LICSW level.
- WI must have one of the following: Certified Advanced Practice Social Worker (CAPSW) or higher license level.
- If licensed in only one state upon hire, must be willing to obtain license in the other state and provide proof of license within 4 months of start.
