Key Responsibilities

  • Provide home
  • based physical therapy to patients who have lost mobility, strength, and functionality due to illness or injury
  • Work as part of a home health team, which may include an RN, LPN, CNA, HHA, OT, and SLP, focused on the patient’s plan of care and goals
  • Act at the direction and under the supervision of the treating Physical Therapist and in accordance with the plan of care

Requirements
