The Nurse Practitioner will provide hands-on primary care focused on senior health, addressing patient needs inside the exam room and considering social determinants of health. They will work in a team-based, value-based care environment, seeing 12-15 patients per day with longer appointment times.
Candidates must be a Board Certified Nurse Practitioner with a valid, unrestricted license in the state of employment, and hold board certification or eligibility in Geriatric, Family, or Internal Medicine. Preferred qualifications include two or more years of experience managing a Medicare Advantage panel and knowledge of Medicare guidelines and HEDIS quality indicators.
