Department: 39602 Aurora Family Service - Supportive Counseling Services Status: Part time Benefits Eligible: Yes Hours Per Week: 20 Schedule Details/Additional Information: Monday thru Friday; 8:00am - 5:00pm, 20 hours per week. Specific schedule will be determined between the employee and clinic.

Key Responsibilities

• solving and resolution of barriers to access to care. Incorporates systemic and solution

• focused approach to case management and care coordination. Serves as integral member of health care team supporting team

• based and industry best practices for professional social work. Adheres to the social work professional code of ethics and demonstrates decision

• based, social work services to diverse patient populations using a cultural humility framework and grounded in principles of trauma
