• Conduct clinical assessments and review health data to evaluate member needs and program eligibility

• Assess member readiness, health literacy, and technology capability

• Engage members in chronic condition support and personalized coaching Health Coaching & Lifestyle Modification:

• Provide individualized telephonic health coaching using motivational interviewing

Requirements

• Monitor progress and adjust plans to meet member needs Required Qualifications:

• Registered Nurse must hold an unrestricted nursing license and reside in a compact state with multi

• state/compact privileges and have the ability to be licensed in all non

• compact states and must reside in the Mountain or Pacific Time Zone.

Benefits & Compensation

• Salary: $60,522 - $142,576/year

• 401(k) with employer match.

Work Schedule

• Full-Time position.

• Remote position - work from anywhere.

• Day shift only - no nights.
