Key Responsibilities
- Improves or minimizes residual physical disabilities of the patient.
- Returns the individual to optimum and productive living within the patient's capabilities.
- Periodically participates with all other home care personnel in patient care planning.
- Responsible for initial assessment, plan of care, maintenance program development and modifications and reassessments every 30 days.
Requirements
- Currently licensed in the state of New Mexico.
- Two years of experience (preferred).
- Lifting Requirements Ability to participate in physical activity.
