About the Role
Join St. Cloud Hospital as a Social Worker | Medical Units in United States. Join the inpatient Care Management team at St. Cloud Hospital and support our Medical Units.
Key Responsibilities
- will have a designated patient caseload and will plan care to effectively meet the patient/family transitional needs including monitoring the length of stay and promoting community resources.
- provide supervision for new grads in-house or will reimburse up to $175/month for external supervision.
Requirements
- Current Licensed Social Worker (LSW) in Minnesota required.
- Bachelor’s or Master's degree in Social Work or Sociology.
- At least two years of experience as a Social Worker, preferably in a healthcare setting.
- American Case Management Association (ACMA) Case Manager-SW Certification required within five years.
Benefits & Compensation
- Pay based on experience and degree Bachelor’s in Social Work starting at $32.
- 09 per hour Master’s in Social Work starting at $33.
- 15 per hour Pay range: $30.
- 73 per hour $3,000 hiring bonus and $5,000 relocation bonus for eligible candidates.
Work Schedule
- Full-time position.
- Day shift only - no nights.
