Key Responsibilities

• As a Nurse Practitioner you will be responsible for patient care within the community to include: Perform initial examinations, including physical exams and patient observations.

• Diagnose and treats physical, emotional, cognitive, and social needs patients.

• Plans and implements individualized medical plans of care with respect to patient needs, reimbursement restrictions, and available resources.

• Provide education and treatment for chronic health concerns with a focus on incontinence, dementia, mobility, depression, constipation, nutritional issues, dehydration, and cognitive impairments.
