About the Role

- The hospital discharge team are responsible for assessing our clients against care act eligibility and also work within the mental capacity act to make best interest decisions where needed.
- The team also complete safeguarding enquiries under section 42 of the Care Act alongside planning for discharge and within the community alongside the review process.
- The post is to support the team in a time of increased pressure and manage the current workload with extra staffing capacity.
- Skills and experience in the hospital setting are essential for these roles.
