About the Role

Join Liquid Personnel as a Social Worker – Hospital Discharge Team Southampton in United States.

Key Responsibilities

The hospital discharge team are responsible for assessing our clients against care act eligibility and also work within the mental capacity act to make best interest decisions where needed.

The team also complete safeguarding enquiries under section 42 of the Care Act alongside planning for discharge and within the community alongside the review process.

The post is to support the team in a time of increased pressure and manage the current workload with extra staffing capacity.

Skills and experience in the hospital setting are essential for these roles.
