The Social Worker provides psychosocial support, assessment, and care planning for PACE program participants within an interdisciplinary team. They facilitate individual and group counseling, resource mobilization, and advocacy to optimize the health and quality of life for geriatric patients.
Candidates must hold a Master’s Degree in Social Work (MSW) and possess at least 2 years of experience in case management or mental health with the geriatric population. A valid BLS certification is required, and bilingual proficiency in Spanish, Chinese, or another language is strongly preferred.
