About the Role

Join Genesis Administrative Services LLC as a Social Worker – M-F Schedule in United States. Overview Rochester Manor seeks a full time Social Worker to join our team! Monday - Friday / 8am - 4:30pm At Genesis Healthcare, we are dedicated to improving the lives we touch through the delivery of high-quality care and exceptional service.

Key Responsibilities

Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well

being. This includes educating staff on the role of social services.

Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.

Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life.

Bachelor's degree in social work or human services field required.
