Employees hired for these roles will be employees of Matrix Home Care, LLC.

Key Responsibilities

• Complete occupational therapy evaluations and reassessments in the home setting.

• Develop and implement individualized treatment plans based on patient needs and physician orders.

• Provide skilled therapy services to improve activities of daily living, upper body strength, coordination, safety, endurance, and functional independence.

• Educate patients and caregivers on home safety, adaptive equipment, energy conservation, transfer techniques, and daily living skills.

Requirements

• Active Florida Occupational Therapist (OT) license in good standing.

• Previous home health experience preferred.

• Experience with evaluations, treatment planning, patient education, activities of daily living training, adaptive equipment, transfer training, and fall prevention preferred.

• Must be reliable, professional, compassionate, and patient-focused.
