Case Manager - Behavioral Health ICMS

Join Southern California Health and Rehabilitation Program (SCHARP) as a Case Manager – Behavioral Health ICMS, Homeless Population in Lynwood, California. Essential Duties: * Receives referrals from DMH and DHS, and assess for eligibility and enrollment to program. * Identify and work collaboratively with DMH, DHS, and other programs to meet client needs.

Key Responsibilities

• Receives referrals from DMH and DHS, and assess for eligibility and enrollment to program.

• Identify and work collaboratively with DMH, DHS, and other programs to meet client needs.

• Assist clients with access to temporary housing, through referrals, until permanent housing placement is secured

• Develop and implement individualized case management services plan.

Requirements

• BA Degree preferred, and two (2) years related experience with the chronically mentally ill and homeless population, and/or in social services related field.

• Excellent written and verbal skills is a must. Must be computer literate and able to compose correspondence notes with correct grammar.

• Must be proficient with Windows operating system and email.

• Familiarization with electronic client management information systems (Clarity/ CHAMP/ Clinivate etc.) Electronic medical record highly preferred.
