Manage a defined patient panel with chronic conditions, performing physical exams and formulating longitudinal care plans. Collaborate with interdisciplinary teams to ensure treatment alignment and provide care across multiple settings including homes and clinics.
Requires a Master's degree in Nursing and at least 2 years of experience as a Nurse Practitioner in cardiology or nephrology. Must hold current state licensure, national board certification, and a valid driver's license for regional travel.
