Advanced Practice Provider (NP) - Hospice (Part-Time)

Key Responsibilities

The Advanced Practice Provider will manage terminally ill patients by conducting physical exams, developing treatment plans, and collaborating with a multidisciplinary team. They are also responsible for educating patients and families, facilitating discharge planning, and ensuring continuity of care.

Requirements

Candidates must be a nationally certified Nurse Practitioner with a current state license and BLS certification. A Master's degree in Nursing (MSN) is required, along with strong clinical skills and the ability to work in a hospice or palliative care setting.
