Public Service Loan Forgiveness (PSLF) Eligible

This position qualifies for federal student loan forgiveness. After 10 years of qualifying payments while working full-time for a nonprofit employer, the remaining balance on your Direct Loans may be forgiven.

Key Responsibilities

  • Participates in multidisciplinary rounds and proactively determines patient needs for discharge transitions including transportation, DME, LTAC, SNF, home health agencies and hospice.
  • Utilizes crisis intervention and problem solving skills to facilitate and enhance patient and family's ability to meet acute and ongoing medical and psychosocial needs.
  • Demonstrates ongoing familiarity with community resources to facilitate positive patient and family outcomes agencies to assure appropriate client access.
  • Maintains effective documentation including tracking and referral information; consistently meets department and UTSW documentation standards.

Requirements

Candidates must have a Master's degree in Social Work from a CSWE-certified institution and hold or be eligible for a Licensed Master Social Worker (LMSW) license in Texas, with temporary licensure acceptable initially. Preferred experience is two years in a related field, and Basic Life Support (BLS) certification is generally required upon hire.
