OT Occupational Therapist- Home Health

About the Role

BAYADA Home Health Care seeks an experienced Occupational Therapist for per diem home health visits serving adult and geriatric patients in senior living communities throughout Woodbridge, Virginia.

Key Responsibilities

• Conduct home visits within designated geographic territories.
• Perform thorough patient evaluations including functional status and ADL/IADL performance.
• Assess cognitive, psychological, and motor functions.
• Develop individualized intervention plans.
• Evaluate home environments for safety and equipment needs.
• Revise occupational therapy care plans as needed.
• Educate patients and families on rehabilitative care.
• Document care management and evaluations using electronic tablets.

Requirements

• Current Virginia Occupational Therapist license.
• Minimum one year recent OT work experience.
• Graduation from accredited occupational therapy program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and computer skills; EMR experience preferred.
