About the Role

Join Eden Health as a Home Health Physical Therapist Assistant (PTA) in United States. A Physical Therapy Assistant will provide in-home services to our patients. * Implement treatment plans ordered by the physician as indicated by the plan of care to all patients on caseload.

Key Responsibilities

  • Identifies risk of injuries or threats to life and health in the patient’s environment (e. g. environmental/safety/mobility risks) and implements appropriate interventions to reduce those risks.
  • Completes accurate and thorough visit notes via Electronic Health Record (EHR) on every patient in a timely fashion.

Requirements

  • Licensed, registered, or certified as a physical therapist assistant, if applicable, by the state in which practicing, unless licensure does not apply and meets one of the following requirements:
  • Passed a national examination for physical therapist assistants.
  • On or before December 31, 2009, meets one of the following:
  • Is licensed, or otherwise related in the state in which practicing.

  • Full-time position.
  • No explicit call, weekend, or night shift information provided.
