About the Role

Join Eden Health as a Home Health Physical Therapist Assistant (PTA) in United States. A Physical Therapy Assistant will provide in-home services to our patients. * Implement treatment plans ordered by the physician as indicated by the plan of care to all patients on caseload.

Key Responsibilities

Identifies risk of injuries or threats to life and health in the patient’s environment (e. g. environmental/safety/mobility risks) and implements appropriate interventions to reduce those risks.

Completes accurate and thorough visit notes via Electronic Health Record (EHR) on every patient in a timely fashion.

Requirements

Licensed, registered, or certified as a physical therapist assistant, if applicable, by the state in which practicing, unless licensure does not apply and meets one of the following requirements:

Passed a national examination for physical therapist assistants.

On or before December 31, 2009, meets one of the following:

Is licensed, or otherwise related in the state in which practicing.

Work Schedule
