About the Role

Join Capitol View Transitional Care Center as a Social Worker, Homecare-Hospice MSW in United States.

Key Responsibilities

provide whole person care addressing bio-psycho-social needs of patients and family.

Provide patients/families with the psychosocial support needed to cope with chronic, acute or terminal illne.

provide education about homecare, palliative care, and hospice.

provide proof of license within 4 months of start.

Requirements

If LGSW, must be actively working on obtaining MN licensure at the LISW or LICSW level.

WI must have one of the following: Certified Advanced Practice Social Worker (CAPSW) or higher license level.

If licensed in only one state upon hire, must be willing to obtain license in the other state and provide proof of license within 4 months of start.
