Key Responsibilities

• As a Licensed Master Social Worker at Eddy SeniorCare PACE, you will: Provide Comprehensive Social Work Services Conduct psychosocial assessments for participants and families.

• Evaluate home environments and determine support needs.

• Deliver ongoing counseling, education, and crisis intervention.

• Support participants adjusting to illness, disability, or changing independence.

Requirements

• Minimum 1 year experience working with older adults; home care experience preferred Strong communication, assessment, advocacy, and crisis

Benefits & Compensation

• Hourly rate: $43.08/hr.

Work Schedule

• Full-time position.

• Day shift only - no nights.
