Key Responsibilities

• Serves as a required member of the Interdisciplinary Team (IDT) and in initial, semi

• annual, unscheduled, and annual assessments; attend morning updates and report changes in participants’ baseline status to appropriate staff on a daily basis.

• Involved in the development and implementation of QAPI activities.

• Ensure that the home care schedule is accurate and that participants are allocated enough time/services. Provide back

Requirements

• Minimum of two (2) years of demonstrated successful experience in home care; prefer in

• home care management experience.

• Experience in the provision of nursing care, including assessment skills, health education, and current nursing treatments and protocols.

• Effective management skills.

Benefits & Compensation

• Salary: $95K - $100K

• 401(k) with employer match.

Work Schedule

• Full-Time position.

• Call responsibilities required.

• No weekends required.

• Day shift only - no nights.
