This position offers weekends off. Work with SAGE GROUP in Arbor Vitae, Wisconsin.
Provide physical therapy services to home health patients to restore function and relieve pain. Collaborate with physicians and interdisciplinary teams to develop and implement individualized treatment plans.
Requires a valid PT license, BLS certification, and 1-2 years of PT experience. Must have at least one year of recent Home Health experience and proficiency in Home Care Home Base (HCHB) charting.
